PACE Ensures Easier Access to Care with New Partnership

PACE of the Southern Piedmont is partnering with Coltrane LIFE Center to create an easier way for seniors to access daily care needs and socialization. PACE participants that reside in Cabarrus County and surrounding areas can now receive adult day health services at Coltrane LIFE Center through new partnership.

The primary location for PACE of the Southern Piedmont is in Mecklenburg County, but the program serves four counties in North Carolina; Mecklenburg, Cabarrus, Stanly and Union. Through this new partnership, participants of the PACE program can now receive adult day health services at a location that’s more convenient for them.  

“It is exciting that PACE services are coming to Cabarrus County. Cabarrus County residents need the comprehensive, quality level of care that PACE provides. This partnership will make services more accessible to older adults right here in our community,” said Susan Caudle, Executive Director of Coltrane LIFE Center. 

Adult Day Health Care (ADHC) provides many benefits for older adults living in the community. ADHC allows seniors living with dementia or other disabilities the opportunity to get out of the house and partake in activities while in a safe environment. While participants are at the center, they receive nursing care, support services, and engage in various activities to increase socialization.

The Program of All-Inclusive Care for the Elderly (PACE) helps seniors with complex medical needs receive the care and services necessary to help them be safe, comfortable and healthy while living in their own community. PACE provides primary medical care, medication, transportation, social activities, rehabilitation therapy, caregiver support, meals, home care & more.
